Abstract

BackgroundAsymptomatic genital herpes is a major public health problem worldwide. For individuals who are infected by genital herpes, the clinical symptoms of genital herpes are usually non-specific and affect the genital area, which is a common site of transmission. Aciclovir (2-5 mg every 3 to 6 hours) is a well-tolerated, highly effective oral treatment for genital herpes, but there are also significant side effects that can occur with aciclovir.

The objective of this study was to evaluate the incidence and clinical features of genital herpes infection using a real-time polymerase chain reaction (PCR) assay and a quantitative immunofluorescence (QIA) assay.

Methods

Study design

This was a case-control study conducted in a tertiary teaching hospital in Spain. The study was a single center, prospective, observational study, with the objective of identifying the association between the incidence of genital herpes in women and their sexual behavior.

Data collection

Participants were sexually active women with genital herpes in Spain between January 2015 and May 2017. The study included participants aged 16 and above who were diagnosed with genital herpes by a swab or a finger test. The study was conducted in the outpatient clinics of the tertiary hospital, in Spain, in the following areas:

  • Infected area: A center for research in women’s health (ICI), Spain. The ICD-10 is the international reference standard for genital herpes infection, and the Infection Control Agency of the World Health Organization (WHO).
  • Infected area: The region covered by the ICI (ICI-II), Spain.

Study participants

Data were obtained through the online case-control study.

Results

A total of 9,848 people with genital herpes were diagnosed with genital herpes in Spain between January 2015 and April 2017. The mean age was 46 (6) years, with a median of 49 years. The median number of sexual partners was 12 (5) and the median duration of symptoms was 11. The median time to genital herpes infection was 9 (5) days. The overall risk of genital herpes infection was higher in women who had genital herpes and who had sex with men (p<0.001) and those who were female (<0.001). The risk of genital herpes was significantly higher among patients who had genital herpes and who were female (=0.002).

The most common sexual behavior was the following: sexual intercourse (n=5, 12.8%); and the most frequent sexual behavior was sexual intercourse (=3, 14.9%). The most common sexual behavior was having sex with a partner (=1, 5.4%); and the most frequent sexual behavior was the use of oral sex (=1, 3.9%).

The most common sexual behavior was the use of oral sex (=5, 12.8%); and the most frequent sexual behavior was having sex with a partner (=2, 8.8%).

The risk of developing genital herpes was significantly higher among patients who had genital herpes and who were female (
